Dear Robert:
Listed for your review and consideration are the items we discussed earlier by phone. Subsequent to the
good faith effort made to address the known deficiencies associated with the pavement on the referenced
project, other areas of concern have developed. They are located as follows:
1. Westbound, west of Lodge Rd.
2. Westbound, west of London Way.
3. Eastbound at Dobecka Dr.
4. Eastbound, west of Highland Meadow.
5. Eastbound at Meadow Run.
Additionally, the requirements outlined in Section Six of the contract documents relative to "Record t_ ~,,

Lastly, the final quantities of some bid items need reviewing. In the original contract are items numbered
40, 43, 47, 50, 51, 80, 81, 93 and 94, in Change Order #2 the 6" schedule 40 bore and the 2" schedule
40 open cut, in Change Order #5 items numbered 36, 39, 41 and 42. Most of these issues are minor and
can be quickly resolved.
