The purpose of this letter is to acknowledge receipt of your claim. We will be investigating the
claim that you have presented. We are also requesting the following documentation that will
assist Nationwide in the investigation of your claim. The information is important to the
processing of your claim; therefore we would appreciate you providing us the following
documentation:
1. Two (2) years of water consumption records. You can normally request these fxom the water
depaslment. Should we hire an investigating engineer they may be able to secure this
information. If they cannot, we will ask you to obtain this information.
2. County's appraisal of your home.
3. All closing papers in connection with the purchase of your home, including any
papers/inspection reports/photos referring to the condition of the house at the time of purchase
and all disclosure documents signed by the seller of the house.
4. Any information that you have concerning foundation repair estimates and/or repairs that have
been done on the home.
5. Receipts, invoices, reports or any other information concerning any plumbing repairs or tests
that have been made to your home.
6. Information concerning any additions to the home, including any contractors or architects
plans.
7. Any other reports/photos referring to the condition of your home, including the foundation.
8. Full details of prior water damage to the home and/or insurance claims made for such damage
to your home.
9. Documents/photos relating to any challenge of the county tax appraisal.
Please forward the requested documentation to me. If these documents are not available, advise
me in writing. In addition, please provide us with any written information provided to you by any
engineer or contractor regarding the condition of or causes of damage to your home.
I will notify you if any additional infom~ation is needed during the investigation of your claim.
