FINAL WAIVER OF LIEN
BEFOREME, the undersigned authority, came in person, Kay Keene, President ,of
Whitewater Construction ~ Inc. , hereinafter called "CONTRACTOR",
and on his/her oath, makes the following AFFIDAVIT:
Upon receipt of $ 9s691.42 pald to the undersigned, on the project known as Oakcrest Barrow
Ditch Drainaoe Improvements. Project ~D~ 04-0~ for w0rkperformed to
thedateof 9-14-05 at the location of oopp~ll, T~as oranyother
property within the Premises including all necessary corrective work; and all materials, labor,
and equipment supplied in connection with said work. CONTRACTOR does hereby release all
mechanics and materialman liens against the property for the work done to the date above.
CONTRACTOR warrants that all suppliers of materials, Sub Contractors, equipment lessom,
and others furnishing materials, labor, and equipment in connection with the work for
CONTRACTOR'S account to the date above hereof have been paid in full and knows of no
claims, liens, or lien rights existing therefore.
