Coppell Rd. to Denton Tap Road) Coppell Project No. WA-01-01 Final Walk Through Punch List Dear Mr. Davis: On the morning of Thursday September 29, 2005, a final walk through of the SH 121 Water Line (Iq. Coppell Rd. to Denton Tap Road) project took place. In attendance were the following personnel: Mr. Larry Davis, City of Coppell Mr. Danny Hightower, City of Coppell Ms. Charlene LaMattina, E.I.T., City of Coppell Mr. Joe Velasquez, Fox Contractors Mr. Douglas Showers, P.E., Birkhoff, Hendricks & Conway, L.L.P. The purpose of the walk through was to identify any items or issues that do not meet the intent of the executed contract between the City of Coppell and Fox Contractors and to test the visible assets to make sure that they are operational for acceptance by the City of Coppell Department of Public Works. The following is a list of deficient items that require attention and re-inspection prior to final close out of this project: 1. Along the full alignment the contractor must establish a stand of grass. It is our understanding that ~ Fox plans to hydro-mulch and begin maintenance of the affected areas before the end of this week. 2.The water denartmo~-t would like a small water llne identifi,-~ti .. o~gn ~ns'/aucu aL m~ ~ ~_ "~%r ~t ' · Sta. 9+40. ~ ' 3. The water department would like curb marking as an indication of the location of the two stub outs located at Sta. 71+65 and Sta. 73+I5. Mr. Larry Davis City of Coppell SH 121 Water Line Final Walk Through September 30, 2005 Page 2 of 2 Once these issues are addressed to the satisfaction of the City of Coppell Inspector, the Contractor must submit their record drawings to the Engineer for preparation of the official Record Drawings. At that time, final quantities shall be calculated and close out documents prepared for submittal to the contractor prior to the processing of final payment. Once a close-out date has been established for the project, the Contractor must contact his bonding company, International Fidelity Insurance Company, to prepare the Maintenance Bond # TXIFSU0284534 to commence for a period of two (2) years from the contract close-out date. If there are any other issues not listed above, please bring them to our attention at your earliest convenience. It was a pleasure to work with you on this water line project.
