Mr. House:
The one year maintenance agreement will expire on December 10,
1993 for the above referenced project. Staff has performed a
bond walk through in this subdivision. A punch list of
deficiencies, which you need to have addressed, has been
formulated.
A copy of the punch list is being provided as an attachment.
Please forward this listing to the contractor(s) responsible for
the paving. The punch list has been forwarded to the Engineering
Department for their records.

Walk Through - Chautaqua Place
The following items were found to be deficient in our recent walk
through:
1. Random crack at center line - Bethel School east of median.
2. Three reflective buttons missing.
3. Random cracks - 302 Bethel School west of driveway & at
driveway.
4.. Random crack in approach - Bethel School at Dakota.
5. Random crack - Dakota at Bethel School (south side).
6. Random crack - Inlet throat Dakota ( east side).
7. Random crack - Eyebrow - Dakota/Dakota (east end).
8. Random crack - 2 Panels west of 308 Dakota.
9. Random crack - 308 Dakota ( west side).
10. Random crack - 300 Dakota.
11. Ponding - Dakota @ Harris approach south end.
Please make sure the contractor(s) receives a copy of this list
in order to ensure that these issues are addressed.
