A waik-thm was conducted on 12/8/95 on the above referenced project. Below are
listing of deficiencies found at that time.
1) Pump out fire vault and clean Out bottom.

Clean out ail four (4) meter boxes and clean out din.
Locate fire hydrant vaive.
Paint sewer clean out lids, red and stencil a red clean out on adjacent curb.
Paint fire vault vaive, fire hydrant vaive red and fire hydrant valve on curb adjacent
to vaive and V on curb adjacent to vault on curb.
Paint main line vaive blue with a V on curb.
Al1 three double checks have to be inspected by a licensed backflow tester.
Stencil a blue W on curb adjacent to meter boxes.
Clean dirt and brick out of sewer clean out boxes and grout inside around pipe.
