Box 540787 Dallas, TX 75354-0787 Re: Sandy Lake Road Project # ST 93-02 Dear Mr. Weber: Attached to this letter is a copy of a memo from Per Birdsall, Sign/Signal Technician concerning damage that occurred to the Heartz Rd./Sandy Lake Rd. signal light on October 7, 1999. As you are aware, damage to the signal light was caused by construction equipment working on the subgrade. The attached memo itemizes the actual damaged parts, labor, equipment and administrative costs associated with placing this signal light back in operation. By virtue of this letter, the City of Coppell is requesting that a check in the amount of $5,032.18 be remitted to reimburse the City for costs associated with placing the signal light back in operation. On 10/7/99 the traffic signal at Heartz & Sandy Lake received a voltage' surge at an estimated voltage of 25KV. This surge, by witness accounts, was initiated by a piece of construction equipment working on the sub-grade of the Sandy Lake Rd. Project. This piece of equipment bumped into a TU transmission line pole that caused the power wires to sag onto our traffic signal span wire. The short resulted in the destruction of many electronic components, and damage or destruction of other components, all of which are listed below. Parts List Cabinet - entire cabinet and components had to be replaced with the exception of the controller and Cabinet Components - Intersection Components - Opticom discriminator cards that should be repairable.
