A walk-thru on the above referenced project was conducted on
and the items below were found to be deficient.

North Drive Approaches & Parking
North Drive Approach, North Side of Drive Approach- Seal Curb return. 10 to 12
foot to the South Rout & Seal Random Crack
Center of Approach- Rout & Seal Random crack that is running Parallel to street
about 15' foot off of street.
South of Drive Approach- Remove concrete spill aim repair curb same location.
North Parking - Rout & Seal Random crack between third parking spaces from the
North off of center line on parking lot.
North Parking - Install additional parking bumper along sidewalk area on East side of
parking lot. Also seal between parking lot & sidewalk same area ADA area.
Rout & Seal Curb & Drive area on South side of center drive 18 feet east of
approach brake.
Repair sidewalk in the ADA Ramp Center Dr. N. Side of Dr.
Remove forms main entrance by Flag Poles.
South Drive approach & Parking sidewalk
Sidewalk N. side of drive way approach seal ADA ramp.
South side repair sidewalk.
Rout & Seal random crack at Chain link fenced area gate back of building.
Rout & Seal random crack at planter island first parking space by side garage door
south east side.
