On the above date, representatives of SAM Engineering & Testing, LP. performed the following testing and inspection
services:
1. Two 1.75" diameter concrete cores (CC) were taken by SE&T from the existing pavement and were tested for
their Length (L)with the following results: ~, ~1~ ,~~
.CC #1 Deceleration Ln. - 121 & Plaza Blvd. AT 227'E. x 6'S. L = 11"
.CC #2 Deceleration Ln. - 121 & Plaza Blvd. AT 69'E. x 4'S. L = 11.25"
2. Based on the above data and our experience, the L of the tested CC ranged from 11" to 11.25" with an average
of 11.25". This range and average meet/exceed the minimum required 10" pavement thickness.
3. The location and number of CC were selected by SE&T and were backfilled with high-early-strength grout
by SE&T after codng.
