There may be some redundancies in the lists.
Please correct the items indicated on each list. Item No. 21 on
page 4 is to be included as a part of Field Change No. 6, as
noted and its cost must be included in your Final Application for
Payment. All other items will be accomplished at L. H. Lacy
Co.'s expense in accordance with the Contract. Once you have
completed work on these items please call me in order that we may
be able to verify compliance as soon as possible without unduly
delaying final acceptance of the project.

- replace brick pavers with holes and cracks in them.

the intersection - pavers need more sand and the eastern
concrete edge joint needs repair. Lower ~he pavers at the
south edge of the crosswalk.

3. Correct drainage "bird bath" in outside lane gutter at south
end of inlet throat blockout Sta. 201+54.5 ±.
4. Inlet/flume at Sta. 201+60 - remove concrete waste.
5. Patch outside curb at bridge approach slab - Sta. 197+99.73.
6. Inlet at Sta. 195+50 - remove mortar slag from throat,
lumber from inside of inlet, clean out interior of inlet.
7. Inlet at Sta. 192+01.96 - remove construction debris from
the inside and repair crack in throat.
8. Patch curb in south end of the south variable height curb
for inlet in item 6. above.
9. Correct drainage "bird bath" adjacent to wheelchair ramp at
northwest corner of Parkway/Denton Tap.
10. Brick paver crosswalk across Denton Tap Road south bound
lanes, north side of the Parkway Blvd. intersection -
replace damaged brick pavers.

repair crack in curb.

intersection - needs more sand in south end of walk and

These items require remedial action to make them acceptable to the City of
Coppell~
The elopes at Andrew Brown Park and along the right-of-way of the adjacent
park property need to be cleaned of construction debris by raking it out
end dispoeing at offeite. It is recommended that buffalogrese eeed be
broadcast on these elopes ee they ere too extreme for the Divieion'e
personnel to maintain. This seed can be obtained from e local source and
provides e Iow growing cover. If funding perm/ts application of wildflower
seed could provide additionel cover end aesthetice.
The water meter double check boxes et Parkway Boulevard need to be reset
to existing grade. The pull box in the median nose on the east segment
of Parkway also needs to be filled to curb height, crowned with the box
set et grade, it appears the bricks need to be regraded due to settling.
The electrical and traffic signal boxes need to he adjusted throughout the
project to grade appearing to have been hit by vehicles installing the
lights.
Additional compaction, filling, and grading around all light poles should
take place in the ~edians of Denton Tap.
Seeding of slopes in and around the creeks and bridges needs to be done
to protect these from erosion. The Grapevine Creek area where rip rap was
added should also be grassed above the rap material.
The paver band at Bethel Road southbound appears to be sliding end may
require additional sand.
The grass in the parkways has not established and the erosion evident
eepecielly north of Sandy Lake needs to be controlled aB mud enters the
roadway. My recommendation is to seed the area with bermudegrase.
