A copy of a letter from Dallas County to Lloyd James of Charles Gojer and Associates, Inc., dated November 26, 1996. Please refer to page four and specifically the hi-lited comment for sheet 23 that states "Dallas County prefers the use of "Y" inlet instead of the grated inlet specified on this sheet." B. A copy of a letter Dallas County dated December 12, 1996 from Mr. James stating the reasons for asking that we reconsider our previous comment about the grated inlet. C. Plan sheets number 1-35 and sheet number 112 which show the Sandy Lake Road construction in your City. Please note on sheet 10 that the subject inlet is right of station 63+00. D. A copy of the engineers final construction cost estimate for your information. The purpose of this letter is to ask for your written desires with respect to the grated inlet. Also please perform a quick review of the plans prior to indicating your approval by signing the title sheet. This is to inform you that I intend to hand deliver the title sheet to various officials for signature prior to the end of next week, i.e., January 24, 1997. City Limits to Dallas City Limit ) Final Plan and Contract Document Review Dear Mr. James: The following comments result from review of your submittal of Sandy Lake Road final plans and contract documents furnished Dallas County on October 30, 1996. SHEET 1, Title Sheet 1. On the index and on the structural and civil standards number the sheets consecutively. Civil standards will need to pick up after sheet 75 in sequence, i.e., 76, 77,. ~ Sheet 2 (E. & Q sheet) 1. Basically this sheet (marked up sheet attached) requires a detailed and thorough check before re-submittal. Several reasons for this include the following: a. Several corrections in item numbers, (Item Number 402 should be Item 4010 "TRENCH SHEETING AND BRACING") etc. are needed. b. Several corrections in descriptions (Item Number 464 should be described as Reinforced Concrete Pipe Cl. III or C1 IV as applicable) not describes as Pipe Sewers. c. In several cases e.g., Lime Treated Subgrade, the items should be listed under Contractor work instead of County work. d. Several items are missing, such as the handrail on the retaining wall at the Bait Shop. e. Most of the descriptions for the various fence items on the E. & Q. sheet do not match the descriptions on plan sheets 10, 11, 14 & 15. Basically, the only fence to be reused is the pipe rail fence. The other fence items (even the temporary farm fences) are to be new materials. The legends on Sheets 10, 11, 14 and 15 do, in fact, accurately depict what is to be done. An example is that the conduit quantities are actually on Sheet 20 not sheet 19. , 3. In addition to the marked up sheet and the above, we are listing additional comments which may indicate a need for changes on the E. & Q. sheets, on the plan or detail sheet showing that item, in the contract documents or in the preliminary and revised construction estimate you provided Dallas County on November 19, 1996. ITEM COM~4ENT, CORRECTION, CHANGE ETC. NO. 160 Specify class 2 which is measured by the S.Y. 340 Asphalt curb - Type B or D HMAC? 423 Blue Book page 631 says ret. wall to be measured by the S.F. Look at page 632 under what is included for payment. 656 Blue Book page 1115 says measured by L.F. or C.Y. 421 Shouldn't this be Item 420 - concrete structures? 450 Railing: Don't we need a pay item for the hand rail on the retaining wall? 514 Shouldn't this be Item 450? 247 Class 5 in Blue Book page 185 measured by the S.Y. 260 Both items should be Contractor work. 496 Sheet 11 says 1 each, sheet 13 says 2 each. Actually, removal of old structure, large is by EACH. Pipe removal payment is by the linear foot. 514 Check to determine if this should be Item 450. 530 Driveway asphalt - what type? E.Q. sheet says sheet 17 - where on sheet 177 550 Item 550 is for chain link fence. Item 552 is for wire fence. Gates need to be described in the Pay Item description by width, i.e., 15' wide or 24' wide. 403 Blue Book page 535 says measured by the S.F. 464 These should be described as "Reinforced Concrete Pipe" not pipe sewers. 474 What type of slotted drain? (Blue Book page 925). Mr. Lloyd James November 26, 1996 Page Three SHEETS 4 & 5, TYPICAL SECTION SHEETS 1. Specific comments to particular Typical Sections are as follows: a. Section 01: 1. Asphalt Curb; Type B or D? b. Section 04: 1. Right of way width must be provided. c. Typical Concrete Pavement Section: 1. Required density of Lime Treated Subgrade is 96% not 95%. SHEETS 6-9 "SUGGESTED CONSTRUCTION SEQUENCING" 1. See our markups (attached) of the "SUGGESTED CONSTRUCTION SEQUENCING', you provided in the contract documents. These markups indicate that several items of work, e.g., Lime and Lime Treated Subgrade previously identified "by County" should be changed to indicate "by Contractor,,. 2. Change narrative on the plans to be consistent with the final version of the narrative in the contract documents. Sheet 10 1. Driveway right of station 66+10; indicate gravel surfacing within the right of way. Remove (drafting) any indication that the drive turns and goes east toward the Harrington Bait Shop. 2. Doesn't the median from 66+24 to 66+50 need to be "solid asphalt"? Sheet 16 1. Provide a reference to sheet where details for conc. apron can be found. Sheet 18 1. Quantities (concrete curb and gutter) are not accurately shown on the E. & Q. sheet. Could they actually be indicated as on Sheet 177 Sheet 19 1. We calculate 53.4 CY concrete for the retaining wall. Do you propose pay item being by CY or by SF? Mr. Lloyd James November 26, 1996 Page Four 2. No pay items are set up for the Type C201 rail on top of the retaining wall shown on the E. & Q. sheet for this retaining wall construction. 3. Need to indicate how miscellaneous items such as 4" perforated pipe, crushed stone and the filter fabric around the perforated pipe, water stop etc. are paid. Blue Book page 631 says retaining wall to be measured by the S.F. Look at page 632 under what is included for payment. SHEET 20 1. Does electrical conduit on the bridge need to be rigid metal instead of the PVC material you have specified? If so, provide a separate bid item in the bridge quantities. Also, provide a mechanism to handle expansion of the conduit on the bridge. SHEET 21 1. Drainage Area map shows the Boat Ramp Road going through the swale. Does this information match the information shown on the Schrickel-Rollins Park Plans? SHEET 22 1. Does Lateral A-1 empty into the "south" RCP only? Plans give impression it dumps into both 33" RCP's. 2. Check the design of Lateral A-1 connection to Line-A. There won't be much left in any of a joint of 33" pipe when the connection is made in the field. SHEET 23 SHEET 24 1. Several references on this sheet indicate design for the flap gate is on Sheet 28. Shouldn't these reference be to Sheet 29? SHEET 26 1. The connection of Lateral B-1 to Line-B will probably need an entire 8' joint of 66" RCP since it's on such a severe skew. Same for Lateral B-4. Mr. Lloyd James November 26, 1996 Page Five SHEET 27 1. Dallas County prefers the use of "Y" inlet instead of the grated inlet specified on this sheet. SHEET 28 1. We require change in design of all laterals. The dimension shown from top of curb to flow line of inlet is only 4.0'. In order to avoid damage to the Reinforced Concrete Pipe during lime subgrade operations we require a minimum dimension of 4.5' 2. For laterals A-4 and A-5 you need a length of at least 12' each before plugging for future connection. Please correct length on E. & Q. accordingly. SHEET 29 1. Provide manufacturer for the flap gate. 2. Indicate how paid or provide separate pay items for the various incidentals such as the gate itself, underdrains, select backfill etc. BRIDGE, GENERAL 1. Does electrical conduit on the bridge need to be rigid metal? If so, provide a separate bid item in the bridge quantities. Also, provide a mechanism to handle expansion of the conduit on the bridge. 2. We believe that the street light foundations on the bridge need to be a separate pay item from the street light foundation set up on the roadways. 3. Should some of the Concrete Traffic Barrier be Type 2 instead of Type 17 4. Regarding end diaframs, several sheets, i.e., sheets 53-59, have a note stating to see details in the plans about end diaframs. We are unable to find details in the plans for end diaframs. Don't you propose using end diaframs (thickened slab)? 5. We note and accept your company's written reply concerning the question about interior diaframs. However, several details or sections in the plans still make reference to interior diaframs. Please correct the opportunity for confusion. 6. On sheet 59; a detail labeled "03 sawed deck joint detail" is indicated. Where is the contractor to saw the deck? Mr. Lloyd James November 26, 1996 Page Six 7. Sheet 75 is missing. Apparently this is a bridge removal sheet. Will this sheet indicate precisely what is to be removed? Also, need to note that care is to be used in removing the existing hand rail and bridge beams that are to become the property of the County. 8. How are preformed joint sealers paid? 9. Any additional or special permits that the contractor must obtain? 10. Armor joints; what is the spacing and number of stud anchors? 11. Several comments for Sheet 39: a. We acknowledge and accept your letter of October 28, 1996 concerning location of Bent 2. b. Show route of proposed storm sewer Line A. c. Show proposed elevations for Bent 2. STANDARDS, GENERAL The following are in addition to specific comments concerning standards and details: 1. STANDARD 1003; cross out details not needed, i.e., detail for bridge approach, if you propose using the design you provided on Sheet 60. 2. STANDARD 1006; cross out items not needed; e.g. "Integral Curb", "Integral Curb and Gutter" "Mountable Curb", etc. that are not applicable to this project. 3. STANDARD 2001; should have expansion material along the back of the inlet also? Why tie the sidewalk to the inlet? Also, does the detail for sidewalk in Coppell match the City of Dallas detail? 4. STANDARD 9009: detail on this sheet for street light foundation is not current. For your information and use, see detail provided in the attachments to this letter. 5. Need a standard (TxDOT) drawing for chain link fence and gates. 6. Need detail for slotted drain. Cross Sections 1. Required scale is 1"=5' horizontal as well as vertical. 2. Sheet numbers at bottom right corner need to be corrected, i.e., 1 of 17; not 1 of 75. Mr. Lloyd James November 26, 1996 Page Seven 3. X-Sections do not accurately depict proposed construction in front of Harrington Bait Shop. 4. Cross sections for the swale excavation on Sandy Lake right of way must be provided. 5. Indicate volumes and quantities for excavation and embankment on each cross section. Construction Estimate 1. Descriptions, units and quantities should be consistent with the E. & Q. sheet and the Bid Proposal Sheets. 2. The attached marked up copy of your November 19, 1996 revised submittal reflects suggested changes in the unit prices, as well as various (not to be considered as all inclusive) comments related to descriptions, item numbers etc. 3. Separate the items and provide subtotal for the various schedules, i.e., Schedule I, Paving, Drainage and Bridge Construction; Schedule II, City of Carrollton Reimburseables; Schedule III, City of Coppell Reimburseables and Schedule IV, Dallas County Road and Bridge District Number One Construction. Contract Documents As a result of your letter dated November 18, 1996 and our meeting discussions of November 19, 1996, on November 22, 1996 I provided you clarifications and instructions relative to the formatting of the contract documents and bid proposal you submitted by letter dated October 29, 1996. The attached marked up copy of several sections (Special.Provision to Items 4, 7 and General Notes) is an addition and supplement to the mark ups provided you on November 22, 1996. Proposal Documents. 1. Make corrections in item numbers, descriptions, units and quantities consistent with the corrections needed for the E. & Q. sheet. 2. Also, need to provide separate categories for Schedule I, Paving, Drainage and Bridge Construction; Schedule II, City of Carrollton Reimburseables; Schedule III, City of Coppell Reimburseables and Schedule IV, Dallas County Road and Bridge District Number One Construction. Mr. Lloyd James November 26, 1996 Page Eight Due to the number of revisions required, you must submit a check set review submittal of plans, cross-sections, contract documents and proposal documents for final review prior to closing out your contract for the referenced project.
