We are transmitting herewith the final punch lists developed from
our final inspections of the project over the past couple of
weeks. Included with the punch list items noted by myself in
your presence are the items noted by the City personnel which
have been left in the punch list form in which I received them.
There may be some redundancies in the lists.
Please correct the items indicated on each list. Ail work is to
be accomplished at the expense of L. H. Lacy Co. in accordance
with the Contract. Once you have completed work on these items
please call me in order that we may be able to verify compliance
as soon as possible without unduly delaying final acceptance of
the project.

Clean and seal joints between the back of curb and median
pavement on all median noses. Remove all construction
debris (including nails, l~mher, tie wire and etc.) from the
median noses and clean and sweep them.
Median nose at Station 91+55.03 - clean and seal joint
between back of curb and median nose pavement and rout and
seal random crack.
Clean and seal redwood joint at Station 91+77.71.
Median nose at Station 94+72.32 - clean and seal
construction joint.
Railroad Marking "R's" are too small, 5'-0" tall vs. 6'-0"
required by the 1988 Edition of the Manual on Uniform
Traffic Control Devices. Markings must be replaced
accordingly.
Median nose at Station 94+95.00 - clean and seal joint
between back of curb and median nose pavement and rout and
seal random crack.
General Note for Entire project: Paint all valve covers and
mark adjacent curb for location purposes.

have poor texture in the outside two lanes.

14. Electric pull box at Station 89+15 needs more gravel.
Railroad Crossing to Grapevine Creek Bridge:
South bound lanes (from bridge to RR):
1. Correct drainage "bird bath" in outside lane gutter of south
bridge approach slab.
2. General cleanup, painting of valve covers needed. Clean and
seal pavement joint at bridge approach slab Station
114+23.77.
3. Correct drainage "bird bath" in outside lane gutter approx.
Sta. 113+75.
4. Sag inlet at Station 111+85.54 - Replace defective manhole
cover.
5. Median nose at Station 110+99.41 clean and seal joint
between back of curb and median nose pavement and rout and
seal random crack.
6. "X" inlet at Station 110+86.4 - cover is stuck. Repair
cover and hole in the interior of the north wall.
7. Light grading required on the Baptist Foundation property to
to redistribute stormwater runoff.
8. "X" inlet at Station 110+52.4 clean out construction
debris.
9. Traffic pull box at Station 109+05.00 - bolt hole is broken.
Repair bolt hole or replace pull box.
10. Double 10' inlet at Sta. 108+50 - remove construction debris
and clean interior. Remove mortar slag from throat.
11. Median nose at Station 105+24.5 - clean and seal the
construction joint between the back of curb and the median
nose pavement.
12. Median nose at Station 104+64.5 - clean and seal the
construction joint between the back of curb and the median
nose pavement.
13. Double 10' inlet at Sta. 103+75.00 - clean out waste
concrete.
14. Traffic pull box at Station 102+90.00 - bolt hole is broken.
Repair bolt hole or replace pull box.
15. North bound ].eft turn lane to Baptist Foundation - epoxy did
not harden under jiggle bar tiles - remove and replace
jiggle bar tiles with proper mixture of epoxy.
16. Median nose at Station 101+17.00 - clean and seal joint
between back of curb and median nose pavement and rout and
seal random crack.
17. Median nose at Station 100+57.00 - clean and seal joint
between back of curb and median nose pavement and rout and
seal random crack.
18. Pull boxes at Sta. 100+23 no pull strings installed.
19. Pull box at Sta. 99+05 - no pull string installed.

20. Embankment slopes at Railroad Crossing are eroding. Repair
erosion damage and plant grass for erosion control.
21. Seal transverse crack on the north side of the railroad
tracks in asphalt portion of the Railroad Crossing.
North bound lanes (from RR to Bridge):
22. 24" culvert north of railroad is either partially
obstructed, has a broken pipe, or is improperly aligned.
Remove obstruction, replace pipe, or correct alignment
whichever is applicable.
23. Repair erosion damage at the first driveway north of the
Railroad Crossing and construct a mountable asphalt barrier
which will keep water from running down the drive while
allowing full access for cars and trucks.
24. Rout and seal sawcut overrun at the south driveway into
Northlake Nursery, and repair the north tie-in of this drive
to the curb.
25. Repair the south tie-in to the curb for the center drive for
the nursery and rout and seal a random crack in the same
drive. Sta. 101+98.5
26. Correct drainage "bird bath" in outside lane gutter approx.
Sta. 103+90.
27. Clean and seal construction joint at south end of the bridge
approach slab, Sta. 113+87.97.
28. Brick pavers on north bound side, south end of Bethel Road
intersection are unacceptable and need to be reworked.
29. Correct drainage "bird bath" in outside lane gutter of south

Grapevine Creek Bridge to Bethel School Road:

Rout and seal the joint between the concrete and asphalt
pavement at the east end of Meadow Creek, Sta. 2+39.08.
5' Inlet in parking lot, Meadow Creek Sta. 0+82, 38' left
remove waste concrete from inlet.
Sta. 118+02 no pull strings in conduit in pull box.
Pull box in east side of median at Sta. 116+75 needs more
gravel in the bottom and pull strings in conduits.

North bound lanes (Bridge to Bethel School Road):

21. Correct drainage "bird bath" in outside lane gutter at
approx. Sta. 128+30 (just north of U.S. Post Office south
driveway).
22. Repair and seal redwood header Sta. 127+90 +/-.
23. Inlet at Sta. 131+12.5 - remove waste concrete and debris
and clean inside.
24. Southeast corner Bethel School Rd./Denton Tap Road - brick
pavers in the Bethel School crosswalk need more sand and
vibration.
25. Inlets on the south side of Bethel School Rd. - east -
remove mortar slag from throats.
26. South Median nose at Bethel School Rd. intersection, Sta.
131+42.05 - clean and seal the construction joint between
the back of curb and the median nose pavement and rout and
seal two (2) random cracks.
South bound lanes (Bethel School Rd. to bridge):
27. Pull box at southwest corner of the Bethel School Rd.
intersection needs to leveled.
28. Correct drainage "bird bath" in outside lane gutter at
southwest corner of Bethel School/Denton Tap intersection.

approx. Sta. 131+20 at paving joint south of intersection
curb return.
30. Fourth concrete pavement panel south of brick paver
crosswalk has an diagonal random crack - rout and seal
crack.

approx. Sta. 118+69 at south end of inlet blockout.
40. South bound left turn lane serving Meadow Creek Drive -
jiggle bar tiles have soft epoxy. Remove all jiggle bar
tiles and reinstall using proper epoxy mixture.
41. Correct drainage "bird bath" in outside lane gutter at
approx. Sta. 118+35.
42. South bound left turn lane serving Meadow Creek Drive -
remove concrete spill in the middle of the lane divider.
43. Correct drainage "bird bath" in outside lane gutter at north
bridge approach slab.
Bethel School Road to Braewood Drive:
North bound lanes (Bethel School Road to Braewood):
(we conducted the inspection in reverse order, i.e., we
inspected the north bound lanes by walking south against traffic
from Braewood to Bethel School Rd. and then crossed to the south
bound side and proceeded north to Braewood)

South bound lanes (from Braewood to Sandy Lake Road):
1. Inlet at Sta. 153+25 - remove construction debris and clean
out interior.
2. Correct drainage "bird bath" in outside lane gutter at
approx. Sta. 154+49 (between drives).

McDonald's south drive - repair joint and clean up spilled
concrete.
McDonald's south drive - south curb broken. Repair curb.
Inlet at Sta. 161+00 - remove construction debris (formwork)
from inside and clean.
North bound lanes (Sandy Lake Road to Braewood Dr.):
5. Inlet at Sta. 161+00 Remove mortar slag from throat and
clean inside.
7. Clean and seal joint at Sta. 160+47.68.
8. Brick pavers in the north drive to Town Oaks Center
(Minyard's) have sunk and must be repaired.
9. Inlet at Sta. 159+00 - Remove mortar slag from throat and
clean inside.
10. Inlet at Sta. 154+72 - remove construction debris and clean
out interior.

FINAL INSPECTION PUNCH LIST FOR GRAPEVINE CREEK BRIDGE
DATE OF FINAL INSPECTION: February 23, 1990

East Bridge Structure:
1. Remove construction debris from the areas between the ends
of the beams and the abutment backwalls; and from the
corresponding areas on the bents.
2. Touch up paint at the northeast corner of the north abutment
barrel.
3. Touch up paint at the northeast side, north face, of the
south bent.
4. Coat conduit and cap the end of the conduit at the southeast
end of the east bridge rail and backfill and compact.
5. Remove lumber and construction trash from the southeast
abutment.
6. Clean all construction trash from the bridge deck,
sidewalks, and gutters, including, but not limited to:
nails, tie-wire, concrete slag, etc. Sweep all bridge deck
and sidewalk surfaces.

Clean all construction trash from the surface of all
concrete riprap including the jogging trail. Sweep same
areas.
Clean rust stains at the light pole bases of all bridge rail
columns with light poles.
Repair the effects of the metal beam guard fence foundation
settlement and backfill to 90% Standard Proctor Density.
Repair bolted connection of the guard rail at the southeast
end of the east metal beam guard fence.
Bridge electrical punch list to follow under separate cover
when electrical inspection is completed.
West bridge structure:

riprap/wingwall and abutment joints.
Clean all construction trash from the bridge deck,
sidewalks, and gutters, including, but not limited to:
nails, tie-wire, concrete slag, etc. Sweep all bridge deck
and sidewalk surfaces.
Clean all construction trash from the surface of all
concrete riprap.
Clean rust stains at the light pole bases of all bridge rail
columns with light,poles.
Repair the effects of the metal beam guard fence foundation
settlement and backfill to 90% Standard Proctor Density.
Backfill, grade, and compact the median area between the
north ends of the bridge structures enclosed by the median
metal beam guard fence.
Bridge electrical punch list to follow under separate cover
when electrical inspection is completed.
Repair damaged concrete riprap as directed the Engineer in
forthcoming drawings.
