Included with the punch list items noted by myself in your presence are the items noted by the City personnel which have been left in the punch list form in which I received them. There may be some redundancies in the lists. Please correct the items indicated on each list. Item No. 21 on page 4 is to be included as a part of Field Change No. 6, as noted and its cost must be included in your Final Application for Payment. All other items will be accomplished at L. H. Lacy Co.'s expense in accordance with the Contract. Once you have completed work on these items please call me in order that we may be able to verify compliance as soon as possible without unduly delaying final acceptance of the project. North bound left turn lane at Skagg's north driveway - replace jiggle bar tiles due to soft epoxy. GENERAL NOTE FOR ENTIRE PROJECT: Remove all construction debris and trash from the medians. 31. Brick paver crosswalk across the north bound lanes on the south side of the Parkway Blvd./Denton Tap Rd. intersection replace brick pavers with holes and cracks in them. 32. Brick paver crosswalk across Parkway Blvd. at east side of the intersection - pavers need more sand and the eastern concrete edge joint needs repair. Lower the pavers at the south edge of the crosswalk. 33. Repair the pavers in the Parkway Blvd. median at the east side of the intersection. 34. Redwood header at east edge of intersection, near the north curb, needs to be repaired. 35. Correct drainage "bird bath" adjacent to wheelchair ramp radius at northeast corner of Parkway/Denton Tap. 36. Repair broken traffic buttons in the radius at the northeast corner of the intersection. 37. Clean and seal redwood header at Sta. 191+75.68. 38. Clean construction trash, debris, and traffic detour barrel out of the ditch adjacent to the north end of the project. 2. Repair erosion damage on the roadway sideslopes along the Park. Remove concrete waste and trash from same. 3. Correct drainage "bird bath" in outside lane gutter at south end of inlet throat blockout Sta. 201+54.5 ±. 4. Inlet/flume at Sta. 201+60 - remove concrete waste. 5. Patch outside curb at bridge approach slab - Sta. 197+99.73. 6. Inlet at Sta. 195+50 - remove mortar slag from throat, lumber from inside of inlet, clean out interior of inlet. 7. Inlet at Sta. 192+01.96 remove construction debris from the inside and repair crack in throat. 8. Patch curb in south end of the south variable height curb for inlet in item 6. above. 9. Correct drainage "bird bath" adjacent to wheelchair ramp at northwest corner of Parkway/Denton Tap. 10. Brick paver crosswalk across Denton Tap Road south bound lanes, north side of the Parkway Blvd. intersection - replace damaged brick pavers. 11. Median nose, Parkway Blvd. west side of the intersection - repair crack in curb. 12. Brick paver crosswalk across Parkway Blvd., west side of the intersection - needs more sand in south end of walk and wheel chair ramp. 13. Correct drainage "bird bath" adjacent to wheelchair ramp in radius at southwest corner of Parkway/Denton Tap. 14. Correct drainage "bird bath" in outside lane gutter at approx. Sta. 189+53. 15. Inlet at Sta. 181+00 remove plywood and construction debris from the interior and repair crack in the throat. 21. Complete modification of rear opening of inlet at Sta. 176+50 as part of Field Change #6 per Engineer's instructions. 22. Remove yellow temporary center stripe in south bound lanes from Sta. 171+00 south to Sandy Lake Road. 23. Repair cracked curb at south header of project (Sta. 166+58). 24. Remove construction debris from deck slab sidewalk and median. Sweep and clean same. 3. Seal all wingwall/riprap joints. 4. Rout and seal all vertical construction joints between new and old concrete riprap slope protection. 5. Remove loose backfill around guardrail foundations, all locations, and replace with backfill compacted to 95% Standard Proctor Density. 6. Repair or replace guardrail attachment at southeast corner of bridge. 7. Bridge electrical punch list will follow under separate cover when electrical inspection is completed.
