The undersigned arrived on site, as requested, to observe the placement of 10 cubic yards of
5000 psi concrete for vault slab, and 100 cubic yards of 4000 psi concrete for slab-on-grade.
Concrete observed on this date appeared to be placed in compliance with project specifications
with regard to temperature, slump, air content, and batch to placement time.
The following samples were obtained for compressive strength testing in our laboratory:
1 set of 4 cylinders (5000 psi)
