Attached are CDC forms for the Sandy Lake Road Project. These forms have been filled out for
your signature. They are identical except for signature to those submitted through the City of
Dallas earlier this year. Those forms accompanied the June 1994 Sandy Lake Road Bridge
Hydraulic Study.
Submission of the signed forms with a cover letter referencing the June, 1994 Sandy Lake Road
Bridge Hydraulic Study should be sufficient. A copy of the Corps approval letter should also
be attached.
