(Inspection performed east I. me to west) The utility patch on Freeport at the approach of Gateway has an area that is spalling and the SW corner of the patch involved a 6" corner of adjacent existing pavement. Both areas require full depth repair. R & R curb Northeast end of median opening and Southeast end. Spalls on existing pavement East of median opening and turn lane needs full depth repair. Clean 1st inlets (N & S) of Gateway on Freeport. R & R approach at the South end (2'±). Seal all fiber board in medians. 7. Clean grout wash from flowline of curb N. of Gateway's approach. 8. Grind to drain ponding at the W. end of Gateway median. 9. Clean RCP in 1st inlet West of Freeport on Gateway. 10. Grout 2nd joint of RCP and at wye in the 1st inlet on S. side. 11. Regrade temporary channel to drain. 12. Grout connections in 54" RCP and 1 joint near pipe reduction. 13. Remove concrete chips and reseal curb on North side near terminus of radius. Page 2. Punchlist 14. Grout Ist joint of RCP in 3rd inlet from Freeport. 15. Seal saw cut at centerline near 4th fire hydrant from Freeport. 16. Seal curb S. side W. of 5th inlet. 17. Lane division button slipped at the 6th inlet-W, bound. 18. Repair corner chip at center joint-4th panel W. of 5th fire hydrant. 19. Clean 6th inlet and RCP and grout 1st joint of RCP (N side). 20. Cut excess ramneck from 1st joint of RCP (Tth inlet) and check 2nd joint and grout if necessary (N. side). 21. Clean and check for positive flow 7th inlet and RCP on S. side. 22. Seal fiber joints in curbs E. of Cotton intersection. 23. Reseal joint in 4th panel W. of 6th fire hydrant. 24. Clean RCP W. of Cotton on S. side and trim ramneck at 2nd joint of RCP. 25. Clean inlet and RCP - N. side W. of Cotton. 26. Seal centerline joint at 5th panel East (Cotton) pavement. 27. Seal curb S. side at 4th panel (Cotton) pavement. 28. Clean and seal curb and joint 1st on Cotton - N. side. of existing Gateway East of existing Gateway utility cut from Freeport Notes - Observations Epoxy used on buttons appears to be excessively brittle. Median opening on Freeport should be wider. Median nose on Gateway should be shortened to provide adequate radius for truck traffic turning West off of Freeport. RG/mp P.O.
