Community No.: 480165 Community: Dallas County, Texas The issues listed below must be addressed before we can continue the review of your request. We must receive the initial fee, $560, before we will begin our review. For this project to be exempt from fees and processing costs, you must provide evidence that the project is primarily intended for flood-loss reduction to insurable structures in the identified flood hazard area that were in existence prior to commencement of construction of the flood-control project. Please submit a copy of either a public notice distributed by the community stating its intent to revise the floodway or a statement by the community that it has notified all affected property owners. · Please provide the following documentation as required under Section 65.12 of the National Flood Insurance Program regulations for the increase in base (10(Y-year) flood elevation (BFE) upstream of the Sandy Lake Road Bridge: 1. Individual legal notice to each affected property owner within and outside of the community, explaining the effect of the proposed action on their property. 2. Certification that no structures are located in areas that would be affected by the increased BFE. · Ms. C. Jean Hansen signed Form 2 of the Application/Certification Forms. The expiration date of Ms. Hansen's Professional Engineering License was December 1994. Please provide evidence this license has been renewed, or resubmit Form 2 signed by another licensed engineer.
