A separate notice will be issued for the bridge after Change Order No. 1 on th~ bridge is approved by the City. Three hundred (300) calendar days are alotted for this project reflecting a contract completion date of November 1, 1986. Liquidated damages are $750.00 per calendar day. The following summary of the pre-construction conference serves as documentation of the items discussed and agreed upon. Any items in the contract documents not mentioned does not relieve the Contractor of those responsibilities. Ginn, Inc. is retained by the City of Co~pell as administrator on this project. Ginn, Inc. will inspect all construction for conformance to plans and specifications. We will also review all payment applications and make recommendations to the Coppell City Council regarding payment. All communication will be with Ashlar Contracting Company and/or Ashlar's general superintendent, and not with any of the subcontractors. Ginn, Inc. will not supervise the construction under any circumstances. Ashlar Contracting Company is responsible for the means, methods, techniques, etc., for the construction of this project. Ashlar is also responsible for supervising all aspects of the constructi.~n as listed below: 1. Maintain a general superintendant on the project at all times to supervise all prime and subcontractor work. e Maintain proper barricades and signage at all times through- out the project site to insure smooth traffic flow and safety around the construction and to inform anyone of any detours available for their use. (City concerned about dropoff between new and existing pavements and access for emergency vehicles.) Notify police and fire and inspector at least 24 hours in advance of any street or intersection closing and provide any detour information. Notify utility companies and coordinate work schedule with them on relocations, excavations, or any conflicts. Comply to whatever requirements they have when working around their utility. 5. Give 24 hours advance notice to the City and/or Ginn, Inc.'s inspector for any testing Or inspections. 6. Maintain access at all times to properties adjacent to the project. 7. Do not park vehicles, construction equipment, etc., on private property. 8. Erect signs at each end of project prohibiting thru truck traffic. (City said that it is already prohibited.) Submit revised construction and cash flow schedules and certificate of insurance prior to beginning any construc- tion on this project. 10. Submit shop drawings on all items except RCP pipe for Ginn, Inc. review. (See specifications on submittal requirements.) 11. Stake out entire storm sewer system prior to the excavation of any pipe or box culverts. Provide inspector with cut sheets. 12. Verify all utilities, horizontal and vertical, prior to installing any storm sewer. Provide inspector with all data. 13. Keep as-built drawings. 14. 15. 16. 17. 18. 19. 20. 21. 22. Obtain all required permits from utility companies, rail- road, etc. City of Coppell requires a street cut permit prior to the installation of storm sewer across Sandy Lake Road. (There should be no charge for this permit.) Any extras or change in the work, in accordance with the contract documents, must come through Ginn, Inc. office for review first. City of Coppell will provide contractor with a water meter for this project. This meter is not to be used on any other job at the same time as this project. (Contractor must provide double check valves.) One tenth of the total amount bid for the field office can be submitted each month on the payment applications. Only the City of Coppell is to operate any water valves on this project. Call 462-9010 for City assistance. Testing (densities, concrete cylinders) will be paid for by the City of Coppell. Re-tests will be paid for by the Contractor. Inspector will take photographs of the site prior to the start of construction to show existing conditions. Batch plant -- must meet all City, state and federal regula- tions for temporary batch plants. Provide Ginn, Inc. with mix design, calibration data, and aggregate analysis for our review and approval. Test pilot beams will be required. Contractor will cast concrete cylinders. Ginn, Inc. will have a testing lab pick up and test them. During the interim, it is the responsibility of the Contractor to care for the cylinders. 23. Do not perform any work at the railroad crossing until further notice. The City of Coppell must first reach an agreement with Continental Pipeline Company. The above information is for your documentation as to what was discussed at the December 17, 1985 pre-construction conference.
