1. Southeast corner northbound phases gravel is needed in pull box.
2. Ground rod for cabinet - place in pull box or knock down past ground level.
3. Pull box southeast corner beside cabinet - need gravel placed in box.
4. Northeast corner present loop pull box for west bound direction - bolt for lid and
concrete work between box & curb or seal with crack seal.
5. Puli box northwest corner in brick pavers - place gravel in pull box.
6. Pull box northwest corner present loop for south bound needs lock down bolt for
Qnazlte box.
7. Eastbound advanced loop puli box needs inter-nut part to lock down bolt (Armorcast
pun box).
8. Southwest corner pull box needs lock down bolts for Armorcast pull box.

1. Southeast mast arm transformer base missing bolt to door cover.
2. Pull box westbound present loops bolt for Quazite pull box.
3. Pull box northeast corner add gravel to Armorcast pull box.
4. Seal all conduits pull box and cabinet with duct seal.
5. Northwest corner replace both pull boxes due to damage - won't lock down.
6. Southeast corner pull box for northbound phases - concrete around bolt - cannot
open.

1. Ground.
2. Pull box for westbound phase pole 5/8 bolt for lid. Check all lock down bolts on all
pull boxes.
3. Pull box for east bound mast arm large amount of pull cord in pull box. Is it needed?
4. Mast arms and signal heads look O.K. We checked eastbound 5 sections and left 3
sections - O.K.
5. Controller cabinet waiting on card rack repair from IDC (Consolidated).
6. All stop bars are located too close to crosswalks.
7. Duct seal in all conduits pull boxes and cabinet.

1. Southwest mast arm touch up paint on transformer base at rust spots.
2. Duct seal in all pull box conduits and cabinet conduits.
3. Westbound advanced loops pull box needs more rock in box.
4. Westbound present loop to home run cable needs to be sealed and more rock in pull
box.
5. Stop bars too close to crosswalk.
General Note:
Would llke to have plans used by Tully on the project, pl~n~ that show conduit runs, etc.
As-builts on each ~tersection and interconnect conduit.
