Dear Mr. Haynes:
At their regularly scheduled council meeting on March 23, 1999, the City of Coppell accepted your proposal in
the amount of $180, 217.54, for the construction of the State Highway 121 Water Line, from Sandy Lake Road
to 675 feet south of Corporate Drive, and awarded you a contract in that amount. We are enclosing five sets of
the following documents:
Bid Schedule
Questionnaire
Bid Affidavit
Affidavit of Safety Record
Standard Form of Agreement
Performance Bond
Payment Bond
Maintenance Bond
These sets have been executed by the City of Coppell. Please complete, sign and return all five sets to this
office. Upon receipt of the executed documents, we will bind them into the contract sets and deliver two sets
to you at the pre-constmction conference.
In addition, please refer to Item 1.26 of the General Provisions of the specifications along with Item 1.26 of the
City of Coppell's Supplementary Conditions to the NCTGOG Specifications, and provide the required
certificates of insurance for this project. We are enclosing a copy of the specifications for your use in
preparing the insurance certificates.
