Pre-qualification Statement of Prospective Bidders. If additional space is needed, please use attachments. b. Pre-qualification Statement of Prospective Bidder's Surety. c. Current Financial Statement submitted by the sole proprietor, partnership or corporation. All three (3) documents must be forwarded to this department at one time. No one form or statement will be accepted individually. If a project is a 'joint venture', both contractors must complete the pre-qualification forms. Concrete Paving - Includes the setting of forms and batching, transporting, placing, consolidation, finishing and curing of concrete used for road surface. 2. Concrete Structures - Includes such items as inlets, box cul culverts, junction boxes, headwalls, wing walls, retaining walls, vaults. 3. Bridge Work - A bridge shall be defined as a drainage structure of over twenty (20') foot span measured from face to face of abutments; the work shall include layout and control; driving piling, pouring piers, columns, caps, abutments, approaches, wing walls, parapet walls and slabs; steel erection; setting precast, pre-stressed concrete members and installation of handrails. 4. Channel Lining - Includes the excavation and forming; and the placing, finishing and curing of concrete. 5. Miscellaneous Concrete - Includes such items as curb and gutter, driveways, valley gutters, sidewalks. 6. Storm Drains - Includes the excavation, bedding, laying and jointing and backfill of reinforced concrete culvert pipe, corrugated metal pipe or cast in place storm pipe. 7. Earthwork - Includes preparing of right-of-way, clearing and grubbing, removal of existing concrete, excavation, embankment, placing of gravel base, lime treatment, Portland Cement treatment, placing of top soil, seeding, sodding and fertilizin. 8. Water and Sanitary_ Sewer Lines and Appurtenances - Includes excavation, bedding, laying and jointing, backfilling and testing.
