On this date, a Terracon technician observed the injection of water at this site. During the
injection process, the following observations were recorded.

Injection pressure 50 to 200 psi at manifold. X
Injection downward in 18 inch intervals. X
Total depth of injection was 10 feet. X
Spacing of injection was 5 feet maximum. X
Injection to refusal at each location. X
Comments: Three (3) injection rigs were used in the morning and four (4) injection rigs were used in the
afternoon. The injection operation was delayed for equipment and injection begin at apporximately
9:30 a.m.
