RE: Permit Application for Work Order #3PFAOCW
The permit application submitted for the referenced service is incomplete and must be
resubmitted prior to further consideration by the Engineering Department. The application
form is incomplete, containing neither contact information or contractor information. No
estimated start or completion date was included, nor were signatures for any responsible
party provided.
The plans provided with the permit do not meet our minimum requirements for plan
submittal. I would call your attention to the city's Right -of -Way Use Ordinance, Section
6 -14 -5, Paragraph C, Number 4, which describes what is required for plan submittal. The
submitted plans are illegible, there is no utility information included, and I found no
discernible plan view that shows where you are relative to city utilities or other franchise
utility's facilities.
Please reference the attached document and prepare your plans per the requirements, then
complete and resubmit the permit application. Your permit will then be reviewed and
