We hereby submit the following specifications and estimates.
Concrete Repair For Median Nose: Approximately 158 Square Feet
Note Advanced will set up street barricades for inside lane closure for specified area.
1 Saw cut failed paving at designated boundaries, and excavate to a depth of 12"
2 Construct thickened ends where needed to enhance load carrying capacity
3 Dowel into ex~stmg pavement, form and install three bars of #4 steel rebar on
12" centers, both ways, place rebar on chairs
4 Concrete shall be 8" thick to street grade with a 2"-6" height at the control ~omt of median top per
plans and specifications. Concrete shall be 4,500 PSI.
5 Apply a coat of liquid membrane curing compound to insure strength development, if needed.
6 Saw cut control~oints as needed.
7 One year warranty
New Concrete Hooded Channel:
1 Form new nose at each end of median. Form 6" curb (48 Linear Feet) each side of median. Leave
out center section by 1.34x48 Linear Feet for colored pattern work.
2 Dowel into adjacent concrete, as needed with #4 rebar on 12" centers.
3 Install #4 steel rebar on 12" centers, each way
4 Pour areas with 6" of concrete and give a light broom finish.
5 Concrete will realize a compressive strength of 3,500 PSI after 28 days.
6 New paving will have full depth expansion ~omts at no more than 60' on center, and
saw cut control points at no more than 20' on center
Patterned Concrete
7 Pour colored/ patterned insert 6" thick, 4,500 psi concrete inside curbs at an approximately
dimension of 1.34 wide by 48 linear feet.
8 One year warranty

1 Insta1146 each yellow flexible delineation tubes with yellow reflective tape at top At
each end. Hetght of tubers will be 36"
2 Install tube bases with two part epoxy system with spacing per plans and spectficattons.

COMBINED TOTAL$ 13,182.00
Plus any applicable taxes, if sales taxes do not apply, customer must provide an
exemption or resale certificate.
Note: Pricing is good for 30 days, but may be accepted at a later date at the sole
option of the company. Changes in materials pricing will impact the final
price of the contract.
Note: Any permit and/or fees required by the city are to be obtained and paid by the
customer.
Advanced Paving Company proposes to furnish all labor, equipment and materials -
complete in accordance with above specifications
Payment is due upon completion
Advanced Paving Company guarantees workmanship and materials for a period stated in
each section, exclusive of mechanical damage, abnormal abuse (i a damage in front of
dumpster area), or chemical spillage
If the proposal meets with your acceptance, please sign the attached copy of our proposal
Upon acceptance it is understood that the proposal, and the terms and conditions set forth,
will constitute full and complete agreement between us

TERMS AND CONDITIONS
1 Owner shall be responsible for locating all buried utility lines and substructures, and to clearly identify and
mark all lines and substructures prior to commencement of work by Advanced Paving Company (For free
line location service, call 1-800-344-8377 at least 72 hours prior to production. By acceptance of this
proposal, as well as these terms and conditions, Owner agrees to indemnify and hold Advanced Paving
Company harmless from all claims, including reasonable attorneys' fees incurred in defending any claim,
resulting from damages to any utility line or substructures, including irrigation lines.
2. Additional insured can be added at no charge Owner to carry tornado, fire and other necessary insurance.
If primary and noncontributory additional wording is required on our insurance, there will be a fee of
$200 00 Advanced Paving Company will carry Workers Compensation Insurance covering its employees
and shall provide public liability and property damage insurance.
3 All work to be completed in a workmanlike manner, according to Better Standard Practices. Any deviation
from above specifications involving additional costs will be executed only upon written orders and will
become an extra charge over and above the estimate
4 Payment is due and payable upon receipt of our invoice. Finance charges of 1 Yz% per month will be
charged on all balances 30 days or more delinquent. 18% ANNUAL PERCENTAGE RATE. If your credit
becomes unsatisfactory before commencement, or at any time during the course of work, adequate security
shall be furnished by you to Advanced Paving Company on its request, or Advanced Paving Company shall
have the right to discontinue work, treat the contract as having been breached by you, and bring suit for the
completed work and for damages by reason of your having breached this contract. In the event that all work
under this contract is not completed in one operation, Advanced Paving Company shall, upon demand, be
paid in full for items of work completed. Reasonable attorneys' fees and cost of collection shall be charged
to you and paid by you, if incurred by us.
5 Advanced Paving Company assumes no responsibility for damages to a customer's parking lot. The
customer guarantees the work area is structurally sound and will hold our repair equipment. Advanced
Paving Company is not responsible for soil volume changes under the pavement structure. You may wish
to employ a professional geo-technical engineer to perform a soil analysis in order to detect the potential for
future soil behavior
6 Towing: Advanced Paving Company will notify the customer in advance and the customer must make
arrangements to have vehicles removed, at their expense, and/or be responsible for all towing charges
incurred Autos not moved, or any other issue that may interfere with the completion of the contractual
scope of work, will result in a $500 00 per hour fee at the property owner's expense.
7 All disputes surfacing under this agreement shall be resolved by binding arbitration in accordance with the
Construction Industry Arbitration Rules of the American Arbitration Association. The prevailing party in any
such arbitration shall be entitled to the recovery of reasonable attorneys' fees and costs expended. This
contract is the sole agreement between the parties affecting this work and shall supersede any other
agreement concerning same.
