On this date, the above technician, representing CMJ Engineering, arrived on-site as scheduled at
the above referenced project to pick up cylinders that were cast on 8/22/06. Upon arrival, the
technician noticed that the cylinders had been turned on their side by what appeared to be heavy
equipment. Three of the cylinders did not have any visible damage. One of the cylinders had
definite visible damage. The contractor was notified of the damages.
