On this date, the above technician, representing CMJ Engineering, arrived on-site as scheduled at
the above referenced project to test concrete on an inlet/junction box located at the intersection of
Oakbend and West Sandy Lake Road. Upon arrival, CMJ was informed by Kyle with Better Inlets
that the area was not ready for concrete to be placed. CMJ's technician notified our dispatcher, who
in turn called John Gay with Tiseo to verify that the previously scheduled concrete placement was not
to occur on this date.
John informed CMJ that he had spoken with Larry Davis at noon on this date to inform him that the
area was not ready for concrete placement at the scheduled time and to call and cancel. CMJ never
received notification of this cancellation.
