Request for Change Order
Mr. Davis we propose the following additional pay Items for the installation of the proposed
"Signal Light Interconnect Conduit". The additional items include all costs for material and labor
necessary to install the 11/2" PVC Interconnect Conduit either by open cut and use of existing
irrigation sleeves, or by bore where required. Removal and replacement of curlex matting where
required. Replacement of disturbed sod, traffic control, and cleanup are included. Ground Boxes
will be billed per each under the original contract Item T-13 "Ground Boxes". The quantities
shown are estimated, actual quantities complete in place will be billed at the unit prices listed
below. Please add 90 additional calendar days to the contract.
11/2" PVC INTERCONNECT CONDUIT: 4600.0 LF @ 7.45 = $ 34,270.00
11/2" PVC INTERCONNECT CONDUIT (BY BORE): 70.0 LF @ 22.00 = 1,540.00
REMOVE EXISTING CURLEX: 2850.0 LF @ 0.65 = 1,852.50
REPLACE CURLEX: 2850.0 LF @ 1.05 = 2.992.50
