necessary to construct the following items of extra work on the
above referenced project:
1) Remove Concrete Sidewalk $750.00
750 SF @ $1.00 /sf
2) Replace 4 Concrete Sidewalk
750 SF @ $2.50 /sf 1,875.00
TOTAL: $2,625.00
To accomplish said additional work, we request an additional 2 days
be added to our contract. Please issue a change order to
accommodate this.
