As you are aware, staff performed a walk through on the above referenced project on May
19 and 20. Below I have listed the punchlist items identified for this project:
* 1. Sta. 4 + 64.60 - Manhole has no rise from cone top.
2. Sta. 0 + 00 Line D - Manhole lid stuck.
3. Sta. 4 + 41.81 - Manhole has no rise from cone top.
4. Fairview - Middle of Street - Flare leaking.
5. Sta. 2 + 47.61 - Manhole needs chimney seal.
6. Matheson Drive - Meter setter leaking.
7. Matheson Circle - Meter setter leaking.
* 8. Sta. 0 + 00 Line G - No rise from cone top.
9. Sta. 8 + 12.90 - No rise from cone top.
10. Sta. 10 + 20.28 - No rise from cone top.
11. Middle of Graham Drive - Sewer cleanout needs reworking.
12. Brickell - Paint MCV cap.
* 13. Sta. 7 + 25.81 - No rise from cone top and no chimney seal.
14. Sta. 2 + 95.12 - Replace meter can.
15. Drexel Drive, near drain - Water service has no water.
16. Brickell Circle - Clean out needs to be reworked.

20. Fairview Circle - Need to replace meter setter.
21. Fairview Drive - Raise fire hydrant.
* 22. Sta. 4 + 52.92 - No rise from manhole cone.
23. Make sure all cleanouts have plugs on end.
24. Sta. 3 + 60.47 - Manhole chimney seal needs refitting.
25. Sta. 3 + 16.37 - Manhole needs chimney seal.
26. Sheffield Court next to manhole - The sewer cleanout needs reworking.
27. Sta. 0 + 00 Line J - Manhole needs chimney seal.

28. Fairview and Parkway - Bring mainline valve to grade.
29. Sta 0 + 00 Line D - Grout manhole ring, install chimney seal and set rainpan.
----30. Matheson Drive - Mainline valve under concrete.
31. Sta. 0 + 00 Line H - Grout incoming pipe.
* I had discussed these items with the contractor and these manhole rings can be sealed
with ramneck seal and then grouted.
Additionally, there are two (2) manhole connections on this project that are improperly
installed. All connections to existing manholes must be core drilled and sealed with cor -n
seal and blocked with concrete. The two connections that were made on this project fail to
meet these standards; therefore, it is necessary to excavate and pour in 3 feet x 2 feet
concrete encasement on outside of connection. It will be necessary to stabilize this blockout
and have construction inspector inspect the repair.
Please ensure that each of these items are corrected prior to acceptance of this project.
