Dear Mr. Caudill:
Enclosed for your information and action is a list of items that require attention prior to the final
acceptance of the referenced project.
1. Restore all disturbed areas according to Pay Items #8, Top Soil; #9 Seeding, and #86 Storm
Water Pollution Prevention Plan including submittal of Notice of Termination.
2. Once restoration is complete, clean all streets.
3. Stake plan grade in area behind Wall #2.
4. Place asphalt transition in Driveway #3 and Driveway #6.
5. Ponding in Driveway #1, # 2 and #7.
6. Saw to the next joint at valves and manholes.
7. Saw and seal the stamped sections.
8. Rout and seal all random cracks.
9. Repair the existing brick wall at the west side of Driveway #12.
10. Ponding at the east and west side of Driveway #12.
11. Replace missing and displaced traffic buttons.
12. Ponding in gutter, Station 46 south side.
13. Ponding in gutter, Station 49+80 (::t) north side.
14. Ponding in gutter, east and west side of Riverchase Drive north of inlets.
15. InstaIl4'x4' drop inlet and necessary piping at low point Station 52 (::t) south side.
16. Ponding on sidewalk, Station 57+60 (::t).
17. Grade existing ditch from alley Station 61 (::t) south side and provide positive drainage to "Y"
inlet.
18. Ponding Station 65+40 (::t) south side.
19. Install lid on inlet D-3.
20. Remove leftover storm drain and PYC pipe.
21. Remove all temporary erosion protection.
22. Remove all temporary traffic control items.
23. Remove forms from valve pads.
24. Replace valves on the customer side of water lines at 1200 and 1204 E. Sandy Lake.
25. Complete headwall J-3 and provide maximum 3:1 slope around opening.

26. Raise lid on storm drain manhole D-7 and grout pipe to the east.
27. Concrete ring on stonn drain manhole E-l1.
28. Grout pipe at inlet F-7.
29. Clean inlet F-3.
30. Grout invert and pipe and concrete ring and cover on storm drain manhole F-9.
31. Raise storm drain manhole F-12.
32. Rework invert in inlet 1-5.
33. Step down ends ofwall13W and 13E as per drawing.
34. Step down east end of Wall #1 as instructed.
35. Place vertical sawed joint in Wall #land Wall #2 per design.
36. Seal between Wall13W and 13E at the connection to the existing box culvert wingwalls.
37. Secure all blocks and clear all weep holes.
38. Provide "as-built" top of wall elevations for Wall #1 and Wall #2.
39. There is a void at the 21" storm drain through Wall #2.
40. Replace nut on MBGF (south side at connection to pedestrian railing) with galvanized nut.
41. Valve on waterline to school is not working.
42. Reset first fire hydrant on north side.
43. Reset irrigation meter to Landmark Nursery so bolts maybe accessed.
44. Replace threaded wing on second fire hydrant on the north side.
45. Reset meter at 1200 and 1204 E. Sandy Lake.
46. Double check is leaking on meter to the Gun Club.
47. All manholes in the street must have 400# rating.
48. Place rain pan and chimney seal in water vault lid.
49. Valve on waterline to Starleaf is not accessible.
50. Replace lid on valve at Riverchase.
51. Raise and place pads on two valves on the east side of River chase.
52. Replace valve stack, extension and pad at fire hydrant north of Riverchase.
53. Place dirt around irrigation meter can at 1421 E Sandy Lake.
54. The last fire hydrant on the north side is leaking.
55. Paint manhole and valve lids and mark locations on curb.
56. Repaint all new fire hydrants.
57. Secure letter of acceptance from Dallas Water Utilities and City of Irving Utilities.
58. Get with Alison Schmidt, Riverchase BOA, 972-582-9644 or 214-906-3158 to resolve any
outstanding issues.
59. Get with CiCi Cameron, Trinity Shores HOA, 972-393-0850 to resolve any outstanding issues.
60. Provide a release from Mr. Park (Megan Choi 214-646-1226) on the construction site lease.
61. Provide a release from the Metrocrest Community Church (972-462-0686) on the stockpile site.
62. Produce record drawings.
I have attempted to bring forth all unresolved construction-related issues on this project. Some of
these items may have already been addressed. Please give each item you careful attention and
please understand that the omission of any obligation you may have under the terms of your
contract is still enforceable.
Thank you for your cooperation during this somewhat difficult endeavor. Keep me informed of any
and all work activities on the site and if you have any questions or need any additional information,
