Dear Mr. Dogett:
The Texas Department of Transportation (TxDOT) has approved your request for construction of
one 30 feet wide driveway and a leading 11 feet wide deceleration lane. This permit also COY,ers
associated utility works within TxDOT right of way as specified on "Authorization to work on
highway right of way within the Dallas Northwest Area", and are detailed on revised
construction plans signed and sealed by you on January 3/2008. Texas MUTCD should be used
on it's entirely to protect SH 121 service road drivers during all phases of construction.
Attached is the approved "Permit to Construct Access Driveway Facilities on Highway Right of
Way", a copy of which should be kept on the construction site at all times.
In addition, please contact TxDOT inspector, John Hicks (469)951-5525 at least 48 hours before any

This work is limited to improvements as specified above and detailed in the attached drawings. Jfin the opinion of TxDOT,
such a installation, construction, modification, or aheration is in conflict with the current highway or planned future highway
improvements, and could endanger the traveling public using said highway,TxDOT shall have the right, after receipt of such
notice, to prescribe such regulations as necessary for the protection of the highway facility and the traveling public using said
highway.
All work on the highway right of way shall be performed in accordance with TxDOT "Standard Specifications fot'
Construction and Maintenance of Highways, Streets, and Bridges". Our firm will use best management practices to minimize
erosion" and sedimentation resulting from the proposed work, and we will revegetate the work area as indicated under
"Revegetation Special Provisions."
Our firm will insure that traffic control measures complying with applicable portions of the Texas Manual ofUnifonnoTraffic
Control Devices will be installed and maintained for the duration of this work (Approval of traffic control plans required by
area/maintenance office). We understand that we will assume all risks associated with om work within the TxDOT right of
way. These risks include, but are not limited to injuries to our workers, and the traveling public, damage to existing utilities
within the area. Our firm is insured and is bonded. Upon request, we will furnish a copy of our current insurance fonn to you.
Refer to attached location map and drawings for a more specific location a description of work activity. The undersigned
representative agrees to comply with the terms and conditions set forth in this notice.

The Grantee MUST Notify tbe Dallas Northwest Area Office at (972)479-9747 at least 48 hours prior to any
construction within the TxDOT Right of Way. In addition, a lane closure notice must be faxed to the Dallas Northwest
Area Office at (972)479-1696 at least 48 hours prior to the setting any temporary lane closures.
