1. ALL MANHOLES: make sure all joints are sealed and grouted around MH walls
2. Repour pad around MH 2 & 3
3. Need to locate MH #5 and mark on curb
4. MH #6 needs rain pan and repour pad, need to grout around pipes at bottom ofMH.
Pipe looks to be lower than flow line and needs curb markings
5. MH #7 needs rain pan and chimney seal missing 1 band; pipe at bottom not lined up
with flow line and pipes at bottom of MH need to be grouted and needs curb
markings
MH #8 needs rain pan, bottom of MH pipe not lined up with flow line, pipes need to
be grouted, and needs curb markings
MH #9 needs chimney seal and there is not pipe for south stub out, pipe at bottom not
in line with flow line, and pipes need to be grouted at bottom of MH
MH #10 pipes need to be grouted and pipe not in line with flow line (looks lower)
MH # 11 pipe at bottom not in line with flow line and pipes at bottom of MH need to
be grouted
MH # 13 needs chimney seal
MH # 15 needs one band on chimney seal
MH # 16 needs grout around pipe at bottom of MH and needs 1 band on chimney seal
MH #17 - need to cut out bottom ofMH where pipe ends
MH # 18 needs curb markings and pipes need to be grouted at bottom of MH and
pipes not in line with flow line

1. Turn first City FH to street
2. Mark ML V on curb at main line valve
3. Paint main line valve lid blue
4. Missing valve
5. Mis-marked FH valve
6. Missing extension on first valve
7. Need pad and extension on 12" off 16" and mark ML V in blue on curb
8. Paint lid blue on BO
9. First FH needs extension and paint yellow
10. Repaint two FH yellow
11. Irrigation test paperwork on DC
12. Paint air valve lid blue
13. Paint FHV and valve lid red to FH
14. FH valve at 240 Dividend cannot work; reset valve stack
15 _ Repair FH at 240, NW trailer
16. Repair leaking FH - 2nd FH east of 240 trailer
17. Repair leaking FH - 3rd FH, east of trailer
18. First FH, SW,leaking and hit; need to straighten
19. Missing ML V on 16" at fIrst FH, SW
20. BO needs to be completed at building #7
21. Clean inside air vault - trash and dirt

26. Air BO in drive not completed; full of water - must dry
27. Replace 5" hit FH, 4th from east side Belt Line Rd, south side of road
28. 2nd irrigation DC needs testing
29. Third FH from Belt Line Rd, north side, leaking
30. BO west of Belt Line needs marking on curb and complete missing pipe
31. Air BO needs to be dry to inspect; holding water - north side of road of Belt Line
32. FH, l5t south of Belt Line not draining; holding in FH
33. Air BO holding water; must be dry
34. 16" water main line valve needs extension and marked on curb
35. FH of 16" north of Dividend leaking and paint lid red and mark on curb and extension
36. 15t valve on 16" main line valve - mark ML V on curb
37. City irrigation meter cans to grade and cleaned and dry
38. Clean up work zone on Belt Line
