Shohre Daneshmand, Acting City Engineer~ SUBJECT: Oak Trail Drainage Improvement Fire Hydrant Leads/Change Order #2 DATE: July 10, 1991 As per the attached referenced correspondence from DeShazo, Starek and Tang, Inc. (D.S. & T.) dated July 8 and July 10, 1991, and M-S Utilities, a utility conflict has been discovered between the existing 6" fire hydrant lead and the grade of the proposed 42" RCP, storm sewer pipe. There is also a potential for a second fire hydrant lead in conflict. In summary, we recommend that change order #2 be approved for the amount of $3,669.60. We also recommend that funds be allocated for a second potential conflict in the amount of $3,700.00 (~), which will then be brought forward under change order #3. Please note that according to the engineer there is a good possibility the second conflict will be avoided, which in that case, change order #3 will either decrease to $500.00 and/or it may not be necessary at all. The contractor needs a decision by Friday, July 12, 1991, or an additional $1,000.00 may incur due to removal of his equipment from the site. We appreciate your timely attention, and should you have any questions, please let me know. As an aside, change order #1 had involved some plan modification only, with no monetary increase, according to U.S. & T. GLS/MSD/bd cc: Alan D.
