Concrete Placement Observation:
The undersigned arrived on site, as requested, to observe the placement of25 cubic yards of
4000 psi concrete for footings at 5xlO box culvert sta.O-,-OO thur sta. 0+25 floor.
Concrete observed on this date appeared to be placed in compliance with project
specifications with regard to temperature, slump, air content, and batch to placement time.
1 set of 4 cylinders (4000 psi) was caST for compressive strength testing in ECS' laboratory.
