Summary of Services Performed (field test data, locations, elevations & depths are estimates) & Individuals Contacted.
The undersigned arrived on site, as requested, to observe:
1. The placement of 60 cubic yards of 4000 psi concrete for box culvert deck at Bethel Rd.
The required test limits were determined to be a slump of 3-6 inches, batch to placement time of
<90 minutes, and concrete temperature range of <90 degrees Fahrenheit.
Concrete observed on this date appeared to be placed in compliance with project specifications
with regard to temperature, slump, and batch to placement time. The air content was slightly
below the project requirements.
The following samples were obtained for compressive strength testing in our laboratory:
SetA:
6 cylinders of 4000 psi concrete for compressive strength testing.
