The undersigned arrived on site, as requested, to observe the placement of 50 cubic yards of
3600 psi concrete: for culven @ Bethel Rd. & Freeport, station # 19+50.
Prior to the placement of concrete, reinforcing steel was observed and appeared to be placed in
accordance with project drawings and specifications. Concrete observed on this date appeared
to be placed in compliance with project specifications with regard to temperature, slump, air
content, and batch to placement time.
I set of 4 cylinders was cast for compressive strength testing in our laboratory.
