The undersigned arrived on site, as requested, to observe:
I: The placement and compaction of soils for Center of intersection of Freeport Parkway and
Bethel Road.
Utilizing the nuclear test method to check the compaction of soils; test results indicated that the
compacted material, at the areas and elevations tested, met or exceeded the project requirements
of 95% of the maximum dry density as obtained in our laboratory using the Standard Proctor
Method (ASTM D-698).
Locations and elevations of all tests are based on stakeout provided by others. We cannot be
responsible for structures located off of the observed engineered pad, misaligned utilities or
stakeout errors causing uncontrolled fill to be placed in structural areas.
The soils observed on this date appeared to be placed in accordance with project drawings and
specifications with regard to lift thickness and moisture content.

The undersigned arrived on site, as requested, to observe:
1. The placement of 90 cubic yards of 3600 psi concrete for paving.
2. The placement of 60 cubic yards of 4000 psi concrete for box culvert top.
The required test limits were determined to be a slump of 4-6 inches, air content of 3-6%, batch
to placement time of <90 minutes.
Trent with Iowa Bridge and Culvert was informed of the excessive slump and air content of the
load sampled. A total of 7 gallons of water was added to the 10 yard load.
The following samples were cast for compressive strength testing in our laboratory:
SetA:
1 set of 4 lab cylinders (3600 psi) for paving
SetB:
1 set of 4 lab cylinders (4000 psi) for box culvert top
