Box 1158(5 Fort Worth, TX 76110 Subject: Addition of Pump No. 4 Village Parkway Pump Station City of C, opp~ll, TX Dear Steve: In response to the owner and engineers request for a chnmology of events as they pertain to the modifications performed on the GE Motor for the above sub~ project please find the requested information below. 1. Osliu Nation received word from AUI on April 21, 2000, that the conduit box on the GE Motor (m/n 5KS513AN3891VI; s/n ORG083004) was in the FI position which was opposite of what was shown on the project drawings. 2. On April 24, 2000, 0slin Nation contacted GB Supply and advised GES of the situation and informed Oslin Nation the Motor should be sent to Shermco Industries in Dalhs, the GE warranty sexyice center, to have the conduit box location changed. Oslin Nation then contacted C & P Pump Services in Grand Prairie to schedule pick-up of the Motor at the jobsite and delivery to 1. The Motor was picked-up and delivered to SheaTnco by C & P Services on April 25, 2000. 4. On April 27, 2000, Shermco advised Oslin Nation the Motor modifications and required testing (See Attachments) were completed and was ready to be returned to the jobsite. 0slin Nation then scheduled pick-up and delivery of the Motor to the Village Parkway jobsite. 5. The Motor was picked-up and delivered back to the jobsite on April 27, 2000. In an effort to bring this matter to a conclusion and not delay the project ccanpletion yon will see 1 have attached the letter of response from GE Supply the Shermco re. pair and ttst report along with the GE dan pack for Motor m/n 5KS5 13AN389M which inc:ludes a Motor data sheet, Motor performance data with speed-torque/current curve, stator frame machining print, and certified print for the Motor in question. I would also advise all panics in-ivy to this information that it is proprietary and should be treated as such. See attsched drav~g to verify. The two loose wires ~n the main ccmdu~t box side of the motor m feetory installed thermocoeples for testing purposes. These er~ usually sixa~hed to the S~xr. We speculste thg they became loosened during hendllug or dudrig the collvereio~ ofthe mOtor e~nduit boxes eom FI to F2 coafigormim. The vent grill c~n be removed m~d these wires my be re-s~tsched wilh cable ties or cut bac& as dose to the srator as possible. GE does mark etatots md rotors with inspector markings.
