Texas 75019 EP Project #: EP-2921 Dear Mr. Witt: Per your request, Envirophase, Incorporated (EP) has performed a Renovation Asbestos Survey on the above location. On Wednesday, December 3, 2008, Ms. Brenda Conley (TDH License # 602825) and Mr. Scott Bass (TDH License # 602701), both Texas Certified Asbestos Inspectors, performed an asbestos survey of the building(s) in accordance with the Texas Asbestos Health Protection Rules (TAHPR) and the National Emission Standards for Hazardous Air Pollutants (NESHAP). The objective of the survey was to identify the most obvious materials for sampling and analysis to determine the presence or absence of asbestos-containing material (ACM). The survey consisted of noting observable materials (materials that are readily accessible and visible in areas accessed by the inspector) which are commonly known to potentially contain asbestos. The survey was intended to identify all sources of asbestos in the property interior and exterior, which may be disturbed during the up-coming renovation. Additional sampling may be warranted should the user's objectives change. The results herein cannot guarantee that no asbestos is present in areas not sampled or areas concealed by physical barriers which restrict access. Property Description The subject property, located at 14901 South Northlake Road, in Coppell, Texas, consists of a single story commercial building containing several individual offices, a common kitchen area, a training room, and mechanical areas. It was vacant and unoccupied at the time of the survey. According to Dallas Central Appraisal District, the building structure was constructed in 1959 and contains approximately 7,505 square feet. The exterior of the building consists of a brick structure with flat built- up roof system. The interior of the building consists of a combination of painted and textured drywall walls. The ceilings consist of exposed acoustical ceiling tile in a metal grid system. Flooring throughout the building consists of vinyl floor tile with areas of exposed concrete in the mechanical rooms. Sampling Methodology A visual inspection and sampling survey was conducted in accordance with current industry standards for a limited asbestos survey to determine the presence of suspect ACM. An initial walk-through was conducted to determine the presence of suspect materials that were accessible andlor exposed. Materials that were similar in general appearance were grouped into homogeneous sampling areas. 1129 North Fielder Road * Arlington, Texas 76012 * (817) 987-3387 * (817) 275-9553 Fax Page 2 Following the walk-through, the inspector collected samples of all building materials defined as suspect Asbestos-Containing Building Material (ACBM). Sampling locations were chosen to be representative of the homogeneous sampling area. Laboratory Methodology Bulk samples obtained from the facility were analyzed in an independent National Voluntary Laboratory Accreditation Program (NVLAP) .Iaboratory using Polarized Light Microscopy (PLM) with dispersion staining (Quantem Laboratories, 2033 Heritage Park Drive, Oklahoma City, OK 73120; NVAP #101959, TDH #30-0143). Samples were analyzed using Polarized Light Microscopy (PLM) (EPA 600/r-93/116) with dispersion staining. The results of this analysis are summarized in the "Findings" portion of this section. Analysis was performed by using the bulk sample for visual observation and slide preparation(s) for microscopic examination and identification. The samples were mounted on slides and then analyzed for asbestos (chrysotile,. amosite, crocidolite, anthophyllite, and actinolite/tremolite, and fibrous non-asbestos constituents). Asbestos was identified by refractive indices, morphology, color pleochroism, birefringence, and extinction characteristics. The same characteristics were used to identify the non-asbestos constituents. The laboratory report and chain of custody is attached to this letter. Findings A total of eighty-four (84) samples were collected, representing 19 homogeneous areas. Of the eighty- four bulk samples analyzed fourteen (14) of the samples tested positive (>1% asbestos) for asbestos- containing materials. A copy of the Laboratory Report is attached. The four options are friable, Category 1, Category 2, and needs determination Category 1 - Materials that are bituminous non-friable and contain more than 1 % asbestos and require removal only when they will be subject to grinding, cutting, sanding or abrading during renovation or demolition activities. **Removal cost estimates are for budgeting purposes only and should not be used as a quote for removal of the materials. It is not our recommendation to remove these materials unless they are beyond repair, or planned demolition or renovation activities will disturb the materials. Estimates are based on recent pricing we have received from contractors performing similar work and may vary from actual prices obtained due to the actual scope of work,quantity of material removed, control measures specified and contractor work loads. A "Friable" material can be broken, crumbled, pulverized or reduced to powder by hand pressure when dry (e.g. structural fireproofing, pipe insulation, ceiling tile, ceiling texture). "Non-Friable" materials include items such as vinyl floor tile, mastics, plastic roof cement, stucco, drywall, drywall joint compound, drywall texture, roofing, and sheet vinyl flooring. A non-friable material may become friable when disturbed or deteriorated. Non-friable Organically Bound (NOB) such as floor tiles (vinyl and asphalt), roofing materials, mastics, and caulking may contain asbestos which is tightly bound to the matrix material and therefore not easily isolated and detected by microscopy. envirOphase ..."_.\..~.,,. Page 3 PLM also cannot detect asbestos fibers less than 0.2 microns in diameter. Because asbestos fibers found in NOB materials are often less than 0.1 microns in diameter, this method can sometimes yield low estimates or even false negative results. EnviroPhase also collected three (3) bulk samples of gypsum wallboard and joint compound from the one-story, brick warehouse building behind the main building. These samples are identified as Sample 1, 2, and 3, on the attached laboratory report. Of the three (3) bulk samples analyzed, none of the samples tested positive (>1% asbestos) for asbestos-containing materials. This building is similar in construction details as the main building and it is assumed to be constructed at the same time, approximately 1959. The interior of the building is open warehouse style with open beam ceilings and sealed concrete floors. A small area has been finished with gypsum wallboard walls and ceiling and appears to be used for file storage. In addition, EnviroPhase also visually inspected a third building located west of the buildings included in this report. The building is constructed of steel beams with metal exterior siding and roof. According to Larry Swift, superintendent with Luminant Energy, it was constructed during the mid 1980's, and contains approximately 25,000 square feet. The building interior is primarily open. construction with exposed steel beam ceilings and walls, with sealed concrete floors. There are finished areas within the building used as a kitchen/lunch room area, an employee locker room, and other small finished areas, totaling less than 25% of the building. These areas were finished with gypsum wallboard and vinyl floor tile. Asbestos sampling for this building was not ordered by the client as part of the Scope of Services. Due to the structure having been constructed mid 1980's; it is possible asbestos would present an environmental concern. Therefore, prior to renovation or demolition of the existing building, EnviroPhase recommends an asbestos survey be performed by a licensed asbestos inspector. According to Mr. Keith Marvin with the City of Coppell, this facility would most likely not undergo any major renovations and therefore the decision to perform an asbestos survey on this particular building was postponed until exact renovation activities are known.
