Dear Mr. Burgy:
On April 22, 2009 a walk- through inspection was conducted for the referenced project. It appears
that all punchlist items have been completed. At this time, the remaining requirements for sign -off
on the CO must be supplied. They are as follows:
1. Provide a utility maintenance bond from a registered surety for 50% of the contract value of
work performed in the public right -of -way and easements for a 2 -year period beginning on the
date of final acceptance. *
2. Inlet protection should be removed in the area of the project. **
* If a bond cannot be secured from a registered surety, you are required to supply a personal or
company check, and the amount will be held for the two year period. Upon a follow -up inspection of
the facilities before the two year term is up, any deficiencies will be noted and you will be given an
opportunity to correct them. You may then request a refund of the bond amount in writing once the
deficiencies (if any) have been satisfactorily cleared up.
** If this task is not done in a reasonable time frame, I will have the inlet work done by others
and will send you an invoice for the work performed.

completion and acceptance, normal wear and tear excepted.

damage occasioned directly by the failure of said materials or workmanship, then this obligation to be void,
otherwise to remain in full force and effect. It is understood, however, that this bond shall not include loss or
damage by failure of workmanship or materials due to hurricane, cyclone, tornado, earthquake, volcanic
eruption or any similar disturbance of nature, nor military, naval or usurped power, insurrection, riot or civil
