application or plat application on the above referenced property located on Sandy lake Road in Coppell ,
Texas. Either agent is authorized to sign any and all applications, furnish any and ail documents, and do
whatever acts necessary to have the property zoned and ptatted as a planned development zone for the
construction, development, and use of the property for a nursing and rehabilitation center. Said agent is
authorized to file all piats, landscaping pians, and all other documents in connection therewith. If

Survey, Abslract No. 886 and the Weston Perry Survey, Abstract No. 1152 in the City of Coppell, Dallas
County, Texas, and being a portion of a tract of land conveyed to Peter Park by deed recorded in Volume
93037, Page 0373, Deed Records, Da11as County, Texas, same being conveyed to Broadmore Health
Reality, Ltd. by deed recorded in Document No. , O$cial Public Records, Dallas County,
Texas and being more particularly descnbed by metes and bounds as follows:
Beg•nning at a 1/2 inch iron rad set with yellow cap stamped "DC&A 1NC" for comer, and being the
Southwest comer of the remainder of said Park tract, aad being in the Noith line of Sandy Lake Road (a
variable widdi right-of-way);
Thence North 89 degrees 21 minutes a2 seconds West, along the North line of said Sandy Lake Road, a
